Eligibility

If you are authorized to work at least 30 hours per week, you are eligible for all of the benefit plans offered. You may also choose to enroll your eligible dependents in many of our benefits. Your eligible dependents include your legal spouse, children, students, and disabled children. Please review information regarding your dependent eligibility.

Please note: dependent spouses who have access to health insurance through their own employment, but who choose to decline that coverage and be covered under Liberty group health plan, will be subject to a bi-weekly premium surcharge of $200.  This will be deducted from the employee’s paycheck on a pre-tax basis.

As a new hire, you are eligible for the following benefits: Medical, Dental, Vision, Voluntary Term Life insurance, Short Term Disability and Long-Term Disability insurance, Accident insurance, Critical Illness insurance, Legal assistance, ID theft, and Pet insurance. You will enroll in these benefits through our Employee Self-Service Benefits Portal.

  • If you decide not to participate in this insurance offered to you at this time, it is still important that you complete the enrollment process and indicate that you are declining (waiving) benefits.
  • If you fail to elect coverage on or before the deadline, you will not have benefits and will have to wait until the next plan year Open Enrollment period to enroll.

To avoid paying double premiums and to ensure that you receive insurance cards prior to your eligibility date, please complete the enrollment process by the week prior to your eligibility date.

Liberty Health provides base life insurance one times your annual salary (up to $50,000) at no cost to you. Please remember to fill out the life insurance beneficiary screen, even if you do not choose any other coverage.

You may change your Medical, Dental and Vision coverage only during open enrollment (typically in February) of each year, or during the plan year only when you experience a qualified change in status, such as marriage, divorce, birth, adoption, placement for adoption, gain other coverage or loss of coverage. The change must be reported to the Liberty Health Human Resources Department within 30 days of the qualifying event. You will be required to provide documentation to support the change. The change in coverage must also be consistent with the qualifying event.

You may discontinue Voluntary Term Life, Short Term Disability and Long-Term Disability at any given time without a qualifying event. If cancelled, coverage will end on the last day of the month.

Helpful Advice for Maximizing Your Benefits Enrollment

  1. To avoid any delays with enrollment, KNOW YOUR USER NAME AND PASSWORD! The Benefits Department does not have access to this information. Contact the Service Desk to get help to reset your password. Their number is 866-999-5447
  2. Have all dependent names, DOBs, and SSNs before starting enrollment. There is only one opportunity to add them during the process, which creates errors if you don’t do it when asked.
  3. Please review the benefits guide to get the plan information before enrollment. The costs and basic plan information are in the guide.
  4. Verify that your address is correct in our system. This is where the carriers send the ID cards. If it needs to be corrected, please update it with your facility so they can send the update to the Corporate office.
  5. Make sure to enroll for the Employee additional life insurance in $20,000 increments ONLY. Other increments create errors. Enroll for the additional spouse life insurance in $10,000 increments.
  6. The enrollment information goes to the carriers via a weekly file sent on Thursdays. If you enrolled on a Friday, the data will go to the carriers on Thursday. Please be mindful of this when asking about enrollment.
  7. Finally, ensure you have your ID card before seeing your provider. If it is an emergency, contact the Benefits Department for assistance. Their number is 910-332-1922 or email the benefits department at benefits@libertyhcare.com.

Qualifying Life Event

Certain life events provide you a 30-day window to elect benefits or change your covered dependents.

What is a Qualifying Life Event? If you experience any of the scenarios below, you may be eligible to update your benefit elections mid-year:

  • Marriage, divorce, or legal separation
  • Birth or adoption of a child
  • Commencement or termination of adoption proceedings
  • Change in child’s dependent status
  • Death of a dependent
  • Loss or gain of other health coverage for you and/or dependents
  • Change in employment status
  • Change in Medicaid/Medicare eligibility for you or a dependent
  • Receipt of a Qualified Medical Child Support Order

Remember, dependent verification will be required.
