Health Reimbursement Arrangement

Liberty Health medical plans do not cover GLP‑1 weight loss medications. To support employees and their dependents who choose to use these medications, Liberty provides a monthly subsidy when they are purchased directly from the manufacturer.

Through a partnership with Flores, Liberty Health offers a Health Reimbursement Arrangement (HRA) that provides a $200 monthly subsidy toward the cost of eligible GLP‑1 weight loss medications, including Wegovy, Zepbound, and Foundayo (new Eli Lilly oral GLP-1 formulation).

This benefit is available to employees and their dependents enrolled in a Liberty Health medical plan.

To participate, your prescribing provider must submit the prescription directly to the applicable pharmaceutical company’s direct‑to‑consumer website (see information below).

As of April 2026, Zepbound is only dispensed in a Single Dose Vial or KwikPen formulation via the Self-Pay option; a separate needle and syringe must be purchased for the Vial option.

The Single-Dose Pen is currently not available via the self-pay option on LillyDirect’s website.

Wegovy is available in either an auto‑injector pen or oral tablet formulation.

How the Subsidy Works

  • If the cost of the medication is $200 or less per month, the HRA card can be used as the form of payment on the pharmacy’s website.
  • Employees enrolled in the Flores FSA can use their existing Flores debit card to purchase the medication on the pharmacy site. If the cost of the medication exceeds $200, the Liberty subsidy will be applied first, with any additional cost deducted from available FSA funds. If FSA funds become depleted, a manual reimbursement will need to be submitted to receive the subsidy if the cost of the medication exceeds $200 per month.
  • Employees not enrolled in the Flores FSA can use the HRA card on the pharmacy’s site if the cost of the medication is below the $200 subsidy. If the cost is more than the subsidy, you will need to pay for the full amount out of pocket and submit a manual reimbursement to Flores to receive the $200 subsidy.

Medications will be shipped directly to your home once payment is complete — no pharmacy visit required.

Employees enrolled in one of Liberty’s medical plans will receive a debit card and welcome materials by mail with instructions for setting up their online account. Please look for a plain, generic envelope containing your new Flores Liberty HRA debit card.

Important Notes

  • The $200 Liberty subsidy applies to 30-day supplies only.
  • The Flores debit card may be used only for the purchase of eligible GLP‑1 medications through the designated direct‑to‑consumer pharmacy channels.
  • The HRA debit card can be used for only one family member. If more than one family member is prescribed a GLP‑1 medication, the additional family member(s) must request the Liberty subsidy through manual reimbursement.
  • If you choose to pay out of pocket, please retain your receipt (including patient name, provider, date of service, and amount paid) and upload it to the Flores website for reimbursement consideration.
